Reading Between the Lines: A Physician's Breakdown of Your Lab Results
You requested your results through the patient portal, and within hours a dense table of numbers, abbreviations, and flags appeared on your screen. Some values are highlighted in red. Others sit comfortably within a range you do not recognize. You close the app and decide to wait until your follow-up appointment—but that appointment is three weeks away.
This scenario plays out millions of times each year across the United States. Laboratory testing is one of the most powerful diagnostic tools available to modern medicine, yet the information it produces is rarely translated in a way that patients can meaningfully use. The goal of this guide is to change that.
Why Lab Literacy Matters
When patients understand their own health data, outcomes improve. Research published in peer-reviewed journals consistently demonstrates that informed patients are more likely to adhere to treatment plans, ask better questions during clinical encounters, and catch discrepancies that might otherwise go unnoticed. Lab literacy is not about replacing your physician's judgment—it is about supplementing it with your active engagement.
With that framing in mind, let us walk through the three panels most commonly ordered in primary care settings: the complete blood count, the comprehensive metabolic panel, and the lipid panel.
The Complete Blood Count (CBC)
The CBC is frequently the first test ordered during an annual physical or when a physician is evaluating fatigue, infection, or anemia. It measures the cellular components of your blood.
Red Blood Cells (RBC), Hemoglobin, and Hematocrit
These three values work together to assess your blood's oxygen-carrying capacity. Hemoglobin is the protein within red blood cells that binds oxygen. Hematocrit represents the percentage of your blood volume occupied by red cells.
- Normal hemoglobin: approximately 13.5–17.5 g/dL for adult men; 12.0–15.5 g/dL for adult women

Platelets
Platelets facilitate clotting. Normal platelet counts range from roughly 150,000 to 400,000 per microliter. Very low counts increase bleeding risk; very high counts may signal inflammation or, less commonly, a bone marrow abnormality.
The Comprehensive Metabolic Panel (CMP)
The CMP is a broader snapshot of your body's chemistry, assessing organ function, electrolyte balance, and blood sugar regulation across 14 separate measurements.
Blood Glucose
Fasting glucose is one of the primary screening tools for diabetes and prediabetes.
- Normal fasting glucose: 70–99 mg/dL
- Prediabetes range: 100–125 mg/dL
- Diabetes is generally indicated at 126 mg/dL or higher on two separate occasions
If your glucose is flagged, your physician may order a hemoglobin A1C test, which reflects your average blood sugar over the preceding two to three months.
Kidney Function: BUN and Creatinine
Blood urea nitrogen (BUN) and creatinine are waste products filtered by the kidneys. Elevated levels may suggest reduced kidney function, dehydration, or high protein intake. Your physician will often calculate your estimated glomerular filtration rate (eGFR) from the creatinine value—an eGFR below 60 mL/min/1.73m² warrants further evaluation.
Liver Enzymes: ALT and AST
Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) are enzymes released when liver cells are damaged. Mildly elevated values can stem from fatty liver disease, alcohol use, or certain medications including statins and over-the-counter pain relievers. Significantly elevated levels require prompt investigation.
Electrolytes: Sodium, Potassium, Calcium
These minerals regulate nerve signaling, muscle contraction, and fluid balance. Most deviations are subtle and correctable, but extreme abnormalities—particularly in potassium—can carry cardiac implications and require timely management.
The Lipid Panel (Cholesterol)
Cardiovascular disease remains the leading cause of death in the United States, and the lipid panel is central to assessing your risk.
LDL Cholesterol
Low-density lipoprotein is often called "bad" cholesterol because elevated levels contribute to arterial plaque buildup. Optimal LDL is generally below 100 mg/dL, though target levels vary significantly based on individual cardiovascular risk factors. Someone with established heart disease may have a target below 70 mg/dL.
HDL Cholesterol
High-density lipoprotein is protective—it helps transport cholesterol away from arterial walls. An HDL above 60 mg/dL is considered cardioprotective; below 40 mg/dL in men and 50 mg/dL in women is associated with elevated risk.
Triglycerides
These fats circulate in the bloodstream and are strongly influenced by diet, alcohol intake, and metabolic health. A normal fasting triglyceride level is below 150 mg/dL. Values above 500 mg/dL may increase the risk of pancreatitis.
Total Cholesterol
While total cholesterol is reported, it is rarely interpreted in isolation. The ratio of total cholesterol to HDL, as well as the individual component values, provides a more clinically meaningful picture.
Understanding Reference Ranges
Every lab report includes a reference range—the values considered normal within the general population tested by that laboratory. It is important to understand that these ranges are statistical constructs, not absolute thresholds of health. A result that falls just outside the range in an otherwise healthy individual may be entirely insignificant. Conversely, a result within the normal range may still be clinically relevant when considered alongside your symptoms, history, and other findings.
This is precisely why a single abnormal value should not cause alarm before you have spoken with your physician. Context is everything in medicine.
When to Seek Prompt Follow-Up
While most lab abnormalities warrant a scheduled conversation with your doctor, certain findings merit more urgent attention. Contact your healthcare provider promptly if your results show:
- Hemoglobin below 8 g/dL or above 18 g/dL
- Potassium below 3.0 or above 6.0 mEq/L
- Creatinine significantly above your personal baseline
